Thank you for scheduling your EMDR Group Consultation with Amber Stiles-Bodnar. You will have received an email confirmation from Amber Stiles-Bodnar (via Calendly) with your scheduled date, time & Zoom link information. Please follow the instructions in the email if you need to cancel/reschedule. 

Below is an EMDR Therapy Case Consultation form to aid in your upcoming consultation. It is not required you send this back in advance or fill it out completely. You may just want to have it on hand during the consultation.
 
A 48 hour notice of cancellation for group consultation is required. Late cancellations/no shows will be charged the full rate. You are responsible for the fee even if you have agency/employer paid consultation. Any exceptions for emergency situations will be individually evaluated. No refunds will be provided. If canceling and rescheduling in time your payment will be transferred to the rescheduled date/time.
